RN Assessments & Personalized Care Plans

 

At Loved Ones Home Care, every client begins with a professional Registered Nurse (RN) assessment to ensure the safest, most accurate, and personalized care possible.
Our RN evaluates the client’s routines, risks, home environment, and daily abilities before any caregiver is assigned.

This process allows us to provide consistent, high-quality care and gives families peace of mind knowing their loved one’s needs are understood from day one.

 

Please note: RN visits, assessments, and follow-up evaluations are billed separately from caregiver services.

What Is an RN Assessment?

 

An RN assessment is an in-home evaluation conducted by our Registered Nurse to determine the level of support a senior needs.
This assessment is required for all new clients to ensure care is appropriate and safe.

 

The RN assessment may include:

  • Review of health history and current conditions

  • Baseline vitals if needed

  • Mobility and transfer evaluation

  • Cognitive and memory screening

  • Fall-risk and safety assessment

  • Behavioral or dementia-related observations

  • Daily routine review

  • Family interview

  • Home safety walk-through


Personalized RN-Created Care Plans

After the assessment, our RN creates a customized care plan outlining exactly how caregivers should assist the client.

 

Care plans may include:

  • Daily routines

  • Personal care instructions

  • Safety guidelines

  • Mobility and transfer steps

  • Dementia-care strategies

  • Emergency instructions

  • Client preferences

  • Caregiver reminders and notes

These plans are updated whenever the client’s condition or needs change.


Ongoing Nurse Evaluations (As Needed)

Our RN is available to provide follow-up visits whenever:

  • A client’s condition changes

  • There is a fall or incident

  • A new safety concern arises

  • Families request an additional evaluation

  • A care plan needs updating

  • A caregiver needs further instruction

Note: RN follow-up visits and evaluations are billed separately.

Caregiver Documentation Oversight

Our RN reviews caregiver documentation to ensure quality and consistency in care.

Oversight may include:

  • Reviewing daily caregiver notes

  • Monitoring changes in condition

  • Making updated recommendations

  • Ensuring the care plan is followed

  • Coaching caregivers as needed
